This isn’t unusual for a field that spent decades in research dormancy — but it means that confident claims in either direction outrun the evidence.

What can be stated with reasonable confidence:

  • The serotonin syndrome alarm is often overstated relative to what research actually shows
  • The combinations that genuinely warrant more caution — MAOIs, lithium — are worth taking seriously
 

The pharmacology gives us a solid foundation. The clinical certainty hasn’t caught up yet.
